Tor writes "We are having massive performance problems on our production SQL Server and don't understand while occassionally a normally efficient stored procedure will blow-up and have a huge number of reads and consume a ton of cpu. I try to execute the sp on my own and it works great (on the same server). Any ideas?"

Are you running maintenance routines to rebuild the indexes, and recalculate the statistics?
