In what scenario – diff and full backups have same size? My guess is excessive fragmentation, any thoughts?

One likely cause is that you have reindexed all the tables with a maintenance plan since the last full backup.

If you are going to use diff backups, you should schedule your full backup to run after any reindexing operations.

That may be the case. We do reindexing in MP after the FULL backup. I will check and comeback.

Does the maintenance plans reindex only those indexes that are fragmented or does it do the same thing it did in 2000 when it did all of them?

If the MP reindexes everything, then you should consider not doing that and instead use a script such as mine (isp_ALTER_INDEX). Check my sig for the link.

You should still schedule the full backup after the reindex operations to reduce the size of the diff backups, even if you only do a partial reindex.
