Try opening UDP 1434 as well. That port is also used by SQL Server communication.

Possibly your app uses a dsn.
make sure along with the server name you put in the port number.
the easiest way to do this is to use control panel odbc dsn setup.
remember though, The DSN saved by this process does not save user name and password.
