In here, SPID 189 seems to be blocked. Is this a case of a dead lock or a blocking issue? How can I tell the difference? What are the ways in which I can avoid this situation?

Any suggestion would be greatly appreciated.

For your first question here is an explanation of the 2 flags.
Trace flags 1204:
Returns the type of lock participating in the deadlock and the current command affect by deadlock.

Trace flags 3605:
Sends trace output to the error log.(If you start SQL Server from the command prompt, the output also appears on the screen).
