In the index structure, it only has three levels: root, intermediate and leaf level, no matter how many pages. my understanding is correct ?

Thanks

Nope. Root, 0 or more intermediates and then the leaf pages. Larger the index, the deeper it'll be and the more levels of intermediates there will be.
