Hello everyone.
I started studying for php web developer this year, and one of my classes is Databases. To be sincere with you, i was not paying attention at classes too much, and now i have to do my first assigment, and im as good as clueless about how to do it. Can you provide me with some tips about it?
Here is assigment:

Create model of database for hospital (one relations diagram is enough). This database needs to have data about staff and patients. Staff can be nurse, doctor, etc. Every patient can be tied for only one doctor. One doctor can have many patients. Every patient owns a chart(idk if chart is a right translation, the book/paper where all of his data and data about illness is stored) where data about his medical condition is stored (this chart can contain more than one disease, but it is not necessary).
After creating model of database , make SQL orders for creating database tables.
Make commands for entering and updating staff and patients in database.
Result should be given in 2 formats:
text file for commands
picture or PDF for diagram
