How do i create a column for phone in sql management studio in the format [0-9][0-9]-[0-9][0-9][0-9]-[0-9][0-9][0-9][0-9]-[0-9][0-9][0-9]-[0-9][0-9][0-9]

Use varchar(however many chars that is) and put a check constraint on it.
