Does using a product like Diskkeeper help a SQL Server database? I'm thinking you need to defrag the database with TSQL and then use a disk defragmenter.

You need to do both. Check out DBCC DBREINDEX and DBCC INDEXDEFRAG in BOL for information about defragmenting a database.
