Basically, I was tasked to find out what user defined roles have permissions on what tables in a specific database. So in essence my report will have to show the user name, their role, name of the table that has permissions under that role and what that permission is. So it would look like this sample:

I have looked in most the the popular publications but to no avail. My deadline is looming over my head and I've still not found a script that can answer this question."

The system procedures sp_helprole, sp_helprolemember, and sp_helprotect can provide a great deal of the information you're looking for. Books Online has more information on these procedures.
