Can this type of layout be done in SQL? A Client is requesting it.

Pro Adress
FIRST NAME ADDRESS
LAS NAME ADDRESS2
MIDDLE NDAME CITY
SUFFIX STATE
SSN COUNTY
DEGREE ZIP CODE
NPI ZIP CODE EXT
ADDRESS TERM DATE

The way I have the format now is that the data is just going accross example....

First Name Last Name Suffix SSN DEGREE NPI ADDRESS....etc.

If anyone can give me some kind of guidence that would be great.

Can you make your data requirement a bit more clearer i.e. data you have and data you expect out. If you want to show a table layout use code quotes like so:


column1    column2
=======    =======
A          B
C          D
E          F
