Don't use maintenance plans - code what you want to do in an SP or SPs?

the sql service account, is it in the same domain as the server ?

Rather than the transaction log, look at the maintenance plan logs. It should give you the action/database that it failed on. Eg re-index, etc.

I had a similar problem, turned out it was failing on a re-index and/or the databases needed to be in 'single' mode.
