Whilst running SQL Profiler on one of my databases, it's returning a host of Event Class - Exceptions, with a Severity of 25. However there's nothing in the either the sql or windows 2000 error logs. Alerts that have been set up to report Severity 25 errors have never been triggered but I can still see them coming through regularly in Profiler.

Any suggestions as to why this is or what next steps to take?

Well have you analyzed what statement runs right before the exception? Also, are you sure that the alert is enabled? I realize that it is setup by default on installation, but did somebody possibly disabled it? If it is enabled, I would suggest figuring out which statement caused the exception, then calling MS to figure out why the alert isn't working.
