Hardware bottlenecks, bad database design, too many indexes (affects modifications), not enough indexes (affects reads), fragmentation, old statistics...

Performance Monitor would be used to check for hardware bottlenecks. sql-server-performance.com is an excellent resource for using this tool.

For indexes, well you'd have to manually analyze things. Fragmentation would be DBCC SHOWCONTIG. Fragmentation is fixed with DBCC DBREINDEX and DBCC INDEXDEFRAG. But DBCC SHOWCONTIG displays the fragmentation level.

Fragmentation level i can check any time or i should do it when many people are not using it.

which one we should run first DBCC DBREINDEX or DBCC INDEXDEFRAG.
